Provision of Minor Adaptations Service

Description

This contract is for the appointment of a supplier for the provision of minor adaptations services. Minor adaptations are low-level, non-structural modifications to a person’s home environment, designed to improve accessibility, reduce risk, promote mobility and support activities of daily living. The provider will be responsible for delivering an end-to-end service, including referral management, appointment scheduling, supply of materials, installation of adaptations, resident communication, completion reporting, record keeping and contract performance reporting.
